I, the authorized representative of the agency identified below, do hereby acknowledge that all rights to electronically transmit, duplicate, retain and use of the programs in the instructional television series specified below granted in our current lease with P Public Media Education LLC terminate August 31, 2007.

I hereby verify that any taped copies of programs which were in our possession, have been erased by recording black over the programs or by destroying the tapes.

In addition, I hereby verify that all schools within our service region as well as central recording sites authorized by our agency, have been notified of the termination of our lease in writing and that any and all videotaped copies of programs must be erased by recording black over the programs or by destroying the tapes - unless Life of Tape Retention Rights (with no duplication or distribution rights) are purchased directly from Public Media Education LLC by individual schools at the current rates published on http://pmi.itmonline.com
